TMJ disorder (CMD in German, Craniomandibuläre Dysfunktion) is a functional disorder of the temporomandibular joint and associated muscles. 20-30% of adults are affected (DGZMK 2024), but only 5% are diagnosed. The rest suffer for years from headaches, tinnitus, and facial pain without knowing the source is the jaw.
10 symptoms of TMJ disorder
Symptoms are often not linked to the jaw because pain «radiates» to other areas.
🦷 Jaw pain: dull, pressing, especially morning and evening. Worse when chewing hard food.
🔊 Clicking, cracking in the jaw: when opening/closing the mouth. Sometimes only on one side.
🚧 Limited mouth opening: normal 40-50 mm between incisors, with TMD often 25-35 mm.
🤕 Temple headache: pressing, starts in the morning, lasts all day. Often confused with migraine.
👂 Ringing in ears (tinnitus): ringing, hissing. In 70% of cases ENT finds nothing, the cause is TMJ.

5 causes of TMJ disorder
TMD often has multiple causes at once, which complicates diagnosis.
Bruxism (teeth grinding)
Most common cause, 60-70% of cases. Nighttime clenching loads the joint and muscles. Treating bruxism with a night guard often resolves TMD too.
Malocclusion
Deep bite, cross bite, missing teeth, ill-fitting bridge or crown. all create asymmetric loading. Treatment: occlusal adjustment or orthodontics.
Stress and psychosomatic factors
40% of cases. Muscle tone rises with stress and anxiety. Hypertonus of masseter, temporalis, pterygoideus, the basis of TMD. Treatment: psychotherapy, meditation, exercise.
Jaw trauma
Car accident, sports injury, blow. Even mild trauma can displace the joint disc (Diskusverlagerung) and cause TMD months to years later. CT or MRI shows the displacement.
Arthritis or osteoarthritis of the TMJ
Especially in people over 50. Cartilage wear. Rheumatoid arthritis can also affect the TMJ. Diagnosed by MRI. Treatment is multidisciplinary: rheumatologist + dentist.

Functional analysis (Funktionsanalyse)
Standardized TMD exam: measure mouth opening, assess deviation, palpate 12 chewing muscle points, test for clicking, evaluate bite. Duration 30-45 min. Cost: 100-200 € private. GKV covers basic exam, not full functional diagnostics.
Instrumental diagnostics (axiography)
Computer-based 3D recording of jaw movement. Tracks condyle paths during opening, closing, lateral movements. 300-500 €. Not GKV-covered.
TMJ MRI
Gold standard for disc evaluation. Shows: disc displacement (anterior, lateral), inflammation, arthritic changes. 250-400 € (GKV covers with clear indication and referral).
DVT (3D X-ray, CBCT)
Cone-beam CT. Shows bony joint condition: arthritic changes, fractures. 200-400 € private. Done by dentist with specialized equipment.
Electromyography (EMG)
Measures chewing muscle activity. Shows asymmetry, hypertonus, coordination issues. Specialized procedure available at CMD clinics. 150-250 €.

TMD physiotherapy

Botox in masseter
Reduces clenching force, lets muscles rest. Especially effective for severe hypertonus. Cost: 250-400 € per session. Duration 3-4 months. Not GKV-covered.
Medications
NSAIDs (ibuprofen, diclofenac) for acute pain 5-7 days. Muscle relaxants (thiocolchicoside, methocarbamol) for 2-4 weeks. SNRI antidepressants (Duloxetin) for chronic pain. Only by physician prescription.
Surgical intervention
Only in complex cases: arthrocentesis (joint flush), discectomy, TMJ replacement. 1-3% of all TMD cases. Performed by maxillofacial surgeon. Cost: 3000-10000 €, GKV covers with confirmed pathology.
